Thyroid hormones appear to play an important role in normal growth and development, especially brain development. The lack of thyroid hormone in the first birth six months leads to irreversible mental retardation (cretinism) and is accompanied by many morphological changes in the brain. front thyroid hormone during two weeks of life prevents the development of these morphological changes upset.

Cretinism is usually classified as sporadic or endemic. endemic cretinism occurs in areas of endemic goiter and is usually caused by extreme iodine deficiency. Goiter may or may not be present. Sporadic cretinism is a consequence of the failure of the thyroid or developed usually due to a defect in the synthesis of thyroid hormones. Goiter is present if a defect in synthesis is the culprit.

While detectable at birth, cretinism often not known until 3-5 months of age. If untreated, the condition eventually leads to changes as severe as to be unmistakable: a dwarf child, with short limbs, passive, resigned, apathetic, mentally retarded.

The face becomes puffy and expressionless, and extended tongue can be extended through the thick lips mouth parted. The skin may be yellow and feel soft, dry and cold to the touch. The heart rate and body temperature is reduced and teething later. The child has poor appetite and frequent constipation and may have an umbilical hernia.

The diagnosis should be done before these changes are visible to give a full treatment effective. In areas of endemic cretinism due to iodine deficiency, iodine replacement is well established before pregnancy. However, replacement of iodine for pregnant women late in the second trimester has been shown to improve neurologic and psychological development of children.
